I could be talking complete nonsense but have you run a virus / malware scan? I know you said that you're pretty sure, but I've heard this happening before due to malware.

I would try adaware and spybot search and destroy. You can find them from a search and they are both free.

As far as I can see its just a .ico file in the home directory? I may be wrong though!
